NorthStar Nursing provides private nursing care to patients in their home. We provide both short and long-term care, NorthStar Nursing can help with the transition home from hospital, surgery center, or rehab facility, or aid in ongoing care with wellness monitoring, chronic care management, medication assistance, wound care, and more. We can help with patient care, physician communication, and provide education to family and caregivers. We also specialize in healthcare planning, helping individuals and families to navigate discussions on advanced directives and emergency planning.
